In general, Dr. Jacob and his partners are considered non-providers for most health insurance plans. We are glad to work with plans that have an out-of-network benefit.
- By seeing Dr. Jacob, you are choosing to receive services from an out of network provider and are aware that you could choose an in-network provider elsewhere.
- The Hospital, Mount Sinai Hospital, is considered in-network with most insurance plans.
- Dr. Jacob is also affiliated with two Ambulatory Surgery Centers, one that is in-network, and one that is out of network.
- During your visit, we will provide you with an estimate of the professional fee for Dr. Jacob to perform your surgery. This estimate does not mean you will be reimbursed by your insurance company. Payment plans are offered upon request. This fee is only for Dr. Jacob.
- Anesthesia, facility fees, laboratory or pathology, or radiology interpretation, if needed, will be provided by separate billing, and will be covered in most cases under your contracted in network rates based on where your surgery is performed.
- From time to time, an assistant surgeon will also submit a bill to your insurance for assistant services. You will be notified of this prior to the surgery.
